Monday-Sunday Available-8hr. & 12 hr. Shifts

Hourly and Visits Available

Responsibilities

For our hospices nurses every day is different. As a hospice nurse with NurseCore you will provide and coordinate nursing care and end of life education. Some of the duties of the hospice nurse include:

  • Performs initial and ongoing nursing assessments
  • Triages patient care problems
  • Develops and maintains patient’s individualized plan of care
  • Performs specialized nursing skills related to palliative and end-of-life care related to managing discomfort and providing symptom relief
  • Consults with and educates the patient and caregiver regarding disease process, self care techniques, end of life care, comfort measures, medication use and side effects
  • Provides psychosocial support to patients and families
  • Provides preparation and support for patient’s death; pronounces deaths in accordance with state regulation, provides post mortem care
  • Coordinates personal care, DME, and obtaining medications and medical supplies                
  • Provides supervision of care provided by CNAs and LVNs and homemakers
Qualifications

  • Current RN licensure with the State Board of Nursing in the State of California
  • One year experience as an RN in the last three years in a supervised setting
  • Current CPR card accredited by the American Heart Association
  • Proof of current Physical within the past year
  • Copy of updated TB results/Chest x-rays within the past year
  • Successful completion of the skills assessment specific to the areas of experience including any specialty areas.
