The Case Manager accomplishes clients' care by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches; monitoring staff performance.

Essential Duties & Responsibilities as the Case Manager includes the following. Other duties may be assigned.

  • Admission Assessments and Visits, including all relevant paperwork
  • Complying with agency time frames for paperwork completion and submission, including turnaround of admission paperwork within 24 hours
  • Tracking and managing supervision of the staff on assigned cases within required time frames
  • Taking call in a rotation; making after hours visits as necessary to insure safe care of our patients
  • Compliance with all regulatory requirements
  • Adherence to all policies and procedures; functioning with good nursing judgment
  • Attending weekly staff meetings
  • Participating in quarterly chart review
  • Ownership and commitment for agency performance improvement, and an active participant in agency performance improvement, including QM activities and chart review.
  • Other projects and duties as may be assigned by the Clinical Director
  • Performs all Registered Nurse duties as described in the Home Care RN Job Description.
  • Licensed as a Registered Nurse through the State Board of Nursing in the State of Texas
  • One year experience as a Registered Nurse in the last three years in a supervised setting
  • Current CPR
  • Proof of current PPD
  • Successful completion of the skills assessment specific to the areas of experience including any specialty areas
