Responsibilities
  • Physical therapy treatment will be prescribed by an authorized practitioner in accordance with facility’s policies and procedures.
  • Responds to requests for service in a timely manner.
  • Coordinates care with patients, patients’ authorized practitioner and appropriate members of the facility.
  • Per facility’s policies, develops and updates the Plan of Care including treatments and goals.
  • Identifies and secures necessary support personnel, supplies and equipment to carry out the physical therapy prescribed.
  • Performs visits as directed by the plan of care within his/her scope of training and experience.
  • Instructs patient, family and health care personnel and supervises aides and therapy assistants performing physical therapy activities with clients.
  • Safeguards facility and client’s equipment and supplies.
  • Assists the physician in evaluating level of function and development and revision of the Plan of Care.
  • Documents patient’s care accurately and timely on the appropriate forms per facility policy.
  • Submits documentation, including assessments, clinical and progress notes, summary and discharge notes, and supervisory notes in accordance with facility policies.
  • Updates knowledge and skills by attending in-services, case conferences and continuing education programs.
  • Respects patient’s rights and maintains confidentiality.
  • The following tasks are also considered essential function of the position:
    • Lifting unspecified weights, depending on assignment requirements.
    • Bending and stooping to prepare equipment and materials for treating patients or managing the patient’s environment.
    • Transferring, turning and positioning patients and assisting patients in walking and moving from place to place.
Qualifications

  • Graduate of a physical therapy program approved by the Educational Section of the American Physical Therapy Association.
  • Currently licensed as a Physical Therapist in Texas.
  • A minimum of one year’s experience as a Licensed Physical Therapist in a supervised setting within the past three (3) years.
