  • Provide skilled nursing care for clients in the home care environment
  • Work with patients’ families to help improve the patient’s abilities
  • Record all care information concisely, accurately and completely, in a timely manner, in the appropriate format and on the appropriate forms
  • Adheres to nursing process and home visitation policies
  • Provide in-home patient care and continuous education to patients and family members
  • Continually assess the clients condition to ensure they are safe in the home environment
  • Respects client’s rights and maintains confidentiality.
  • Provides the Clinical Director reports of client status.
  • Implements the nursing process in the development of an individualized Plan of Care for clients with necessary revisions based on client needs.
  • Provides Advance Directives and Client Rights information to clients.
  • Counsels the client and family in meeting nursing and related needs.
  • Documents supervisory visits in accordance with agency policies and procedures.
  • Safeguards company’s and client’s equipment and supplies.
  • Prepares clinical, progress, and summary reports per agency policy and procedure.
  • Receives verbal and/or written orders from physician(s).

Routine Functions:

  • Performs routine nursing duties including, but not limited to, client assessment, development and revision of Plans of Care, implements nursing procedures, evaluates client progress, accurate, timely, and complete documentation of client care.
  • Notifies physician and Clinical Director of any changes in client’s condition or any unusual symptoms.
  • Performs the following nursing procedures where training, and the state Nurse Practice Act permits, including but not limited to:Medication administration per physician orders.
  • Starting IV’s and administration of IV medications.
  • Administering respiratory medications.
  • Changing dressings (sterile or clean).
  • Supervising nursing and other health care member staff.

  • Graduate of accredited nursing school
  • Current RN License in the state of Arizona
  • IV experience required, certification preferred
  • 1 year of experience as an RN  (must be within in the last 3 years)
  • Current CPR
  • Proof of current PPD
  • Successful completion of the skills assessment specific to the area of practice
