Responsibilities

• Our Field Case Managers plan, implement, coordinate, monitor, and evaluate services and outcomes to maximize the health of the patients.  
• Coordinates, monitors and ensures that appropriate home health care services are provided to patients across the continuum of care.  
• Promotes effective healthcare utilization, monitors health care resources and assumes a leadership role within the interdisciplinary team to achieve optimal clinical and resource outcomes for all patients.  
• Works directly with the patients in the field, i.e., patient’s home, provider's office, hospitals, etc. while collaborating with management to assess, plan, implement, coordinate, monitor and evaluate services and outcomes to maximize the health of the patients.

Qualifications

• Licensed as a Registered Nurse through the State Board of Nursing in the State of Florida.
• One year experience as a Registered Nurse in the last three years in a supervised setting
• Current CPR
• Proof of current physical/PPD
• Successful completion of the skills assessment specific to the areas of experience including any specialty areas.
